We provide the following information as a courtesy. However, you should contact your health insurer to inquire regarding your specific insurance plan’s benefits.
We are contracted with the following health insurers:
We are an in-network provider for all DMBA plans.
We are an in-network provider for all PEHP plans.
We are an in-network provider for all UUHP plans.
We are an in-network provider for all EMI plans.
We are an in-network provider with Blue Cross Blue Shield.
We are an in-network provider with SelectHealth commercial (i.e. insurance obtained through an employer) plans.
Out-of-network insurers
We are happy to courtesy bill all major commercial insurers as an out-of-network provider. We regret that we are unable to see patients enrolled in Medicare or Medicaid at this time.
